Join our fully remote Hospital at Home program as an Acute Care Nurse Practitioner (AGACNP) delivering acute, hospital-level care to patients in their homes across California. After just 2 days of in-person onboarding in Walnut Creek, CA, you'll work entirely from home, conducting comprehensive virtual assessments, managing acute care plans, and coordinating with a multidisciplinary hospitalist team and home health providers to deliver seamless care transitions.
What You'll Do
• Perform comprehensive virtual assessments to evaluate patient conditions, history, and symptoms
• Review and interpret medical records, labs, and imaging to guide clinical decisions
• Determine patient eligibility for the Hospital at Home program using established clinical criteria
• Develop individualized care plans in collaboration with a multidisciplinary hospitalist and home health team
• Monitor and manage patients remotely, coordinating timely in-home services and follow-up
• Communicate with patients, families, and caregivers to ensure understanding of at-home care plans
• Coordinate care transitions with primary care physicians and specialists
• Document visits accurately in EPIC in compliance with regulatory requirements

Qualifications
• Active California NP license required
• Must reside in California
• AGACNP certification preferred
• Valid DEA license required
• Minimum 5 years of clinical experience in internal medicine, emergency medicine, or hospitalist settings
• Telemedicine or virtual care experience strongly preferred
• EPIC proficiency required
• Strong virtual assessment, communication, and care coordination skills
