About the position
This position is responsible for researching, developing, and implementing a complex care position with a focus on individuals who are living with brain injury, Alzheimer’s disease and related dementias (ADRD). The position will serve as a lead case manager and will report to the Senior Long Term Care Division. The position will be responsible for designing, developing, and implementing a complex care management initiative and related ADRD policy frameworks, with support from Department leadership. The member population will include significantly complex medical conditions, neurocognitive and mental health co-morbidities. This position will be responsible for managing multiple highly complex cases simultaneously while providing oversight for placement stability.
Responsibilities
• Researching, developing, and implementing a complex care position with a focus on individuals who are living with brain injury, Alzheimer’s disease and related dementias (ADRD).
• Serving as a lead case manager.
• Designing, developing, and implementing a complex care management initiative and related ADRD policy frameworks.
• Managing multiple highly complex cases simultaneously.
• Providing oversight for placement stability.
Requirements
• Bachelor’s degree in health care, human services, business, or public administration.
• Four years’ experience in case management, including complex cases.
• Knowledge of Brain Injury, Alzheimer's disease and related dementias, person-centered care principles, and care models.
• Knowledge of program development, policy drafting and evaluation.
• Knowledge of regulatory requirements for Category D assisted living facilities.
